Hospital Sisters Health System (HSHS) Medical Group, launched in 2008, is a growing network of clinical providers: physicians, nurses, medical professionals, multi-specialty groups, single specialty groups, solo practitioners and medical schools in Illinois and Wisconsin that are working alongside HSHS to create a new, integrated approach to health care delivery. (St. Francis is one of the 13 hospitals in the HSHS healthcare ministry.)
The Medical Group is a critical component in HSHS' Care Integration strategy, which focuses on enhancing the relationships between patients and providers through a seamless, integrated network of care.
To help the HSHS Medical Group provide patient centered, physician-guided and cost efficient health care, a MyHSHS Healthy Medical Home web portal went live in November 2009. It can be accessed via myhshs.org or hshsmedicalgroup.org. This micro site is available to existing and new patients of Priority Care Springfield, Litchfield Family Practice Center, and Macoupin Family Practice Centers; the collaborative partners and test (beta) sites for this new initiative.
Through MyHSHS.org, patients and their family members can complete a Personal Health Record and send it directly through a secure web connection to their physician's office. A health information library, symptom checker and the ability to electronically request appointments and medication refills, as well as billing inquiries, also are available through MyHSHS.org.
The Medical Group's Healthy Medical Home initiative also includes access to a Health Information Line staffed by experienced nurses. The nurses answering the Health Information Line provide callers with confidential access to general health information, physician/specialist referrals and information about physicians and programs associated with HSHS hospitals' St. Francis in Litchfield and St. John's in Springfield.

"With telephone counseling, on-line portal access and more tools for members to access, Healthy Home is being designed to help patients and their families achieve the healthiest life possible."
