Records & PrivacyOur Health Information Services Department (which includes the Medical Records Department) is committed to protecting your privacy and giving you timely, accurate access to your medical records. The following information briefly outlines the steps for requesting your medical records and provides you with an overview of our privacy practices.
Requesting Your Medical Records
Regardless of who is requesting the information - the patient, insurance companies or others - a consent form must be signed by the patient allowing their medical records to be released. To shorten your wait time, we ask that all patients call ahead when requesting their records so we can have it ready for you when you get here.
If you are taking your records to a physician, there is no charge for this service. For any other reason, you will be charged a minimal fee. Insurance companies or others requesting your medical records will also be charged appropriately and require a signed consent form from you for the records to be released.
Privacy Practice
St. Francis Hospital strives to maintain the confidentiality of your protected health information which includes your medical records and other documents used in your care and treatment as established by law.

If at any time you feel your rights have been violated, please call our Director of Quality Services at (217) 324-8596. You may also mail your complaint to:

All complaints will be investigated. The Director of Quality Services can also provide you with a written complaint form for the U.S. Department of Health and Human Services, Office of Civil Rights. Under no circumstances will you be penalized or retaliated against for filing a complaint.
